First Visit
Your initial visit will include an exam, during which we will discuss whether you or your child would benefit from braces or other orthodontic treatment. It is the first, but certainly not the only, opportunity you will have to ask any questions you may have. We may take an x-ray during this visit if we need specific information that can only be obtained radiographically.
It is a good idea to bring your current insurance card. If you don't have one, try to bring your subscriber number, group number, and insurance phone number. This will save time and allow us to help you in the processing of your claim if you proceed with treatment. Remember, your initial consultation is at no cost to you.
Important: All patients under the age of 18 years must be accompanied by a parent or legal guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medications.
If your dentist has taken X-rays they feel would be helpful, please have them forwarded or emailed to this office.
